BRIEF DESCRIPTION:

Holstep Health is seeking a compassionate and experienced Q Health AOD Counsellor and Case Worker to join our Generalist Counselling Team.

The Q Health program provides specialist counselling and case work support to members of the LGBTIQA+ community who are experiencing, or have experienced, concerns relating to alcohol and other drug (AOD) use. This position provides tailored, queer-sensitive and affirmative support that responds to each client's individual needs.

YOUR NEW ROLE:

As the Q Health AOD Counsellor and Case Worker, you will provide specialist counselling, and case work support including care planning to LGBTIQA+ clients experiencing concerns related to alcohol and other drug use.

Working collaboratively with clients, families where appropriate, colleagues and external service providers, you will support positive client outcomes through person-centred, inclusive and evidence-informed practice. 

YOU WILL BE RESPONSIBLE FOR:

• Provide queer-sensitive and affirmative counselling, case work and care planning.
• Conduct bio-psycho-social assessments of AOD-related risk and protective factors.
• Develop individual care plans collaboratively with clients.
• Use appropriate assessment and outcome-measurement tools.
• Coordinate internal and external referrals based on identified client needs.
• Contribute to group interventions relating to AOD use where appropriate.
• Maintain accurate client records, assessments and care plans.
• Participate in professional development and clinical supervision.
• Contribute to program targets and KPIs.

You will also build relationships with health, social service and education providers and work collaboratively to identify service gaps, facilitate referrals and support shared care.

WHAT YOU NEED TO SUCCEED:

To be successful in this role, you will have:

• Relevant tertiary qualifications in Social Work or an equivalent discipline as determined by Holstep Health.
• A minimum of two years' experience in the AOD or mental health sector.
• Demonstrated ability to provide effective therapeutic interventions and strategies that support positive client outcomes.
• Demonstrated ability to provide queer-sensitive counselling and/or case work.
• Experience and knowledge in working with trans and gender-diverse clients.
• A commitment to working with people from the LGBTIQA+ community.
• Strong organisational skills and the ability to manage competing priorities and deadlines.
• Initiative, creativity and the ability to contribute positively within a supportive team environment.
